Acknowledgement of Risks & Obligations
Prior to the participation in any club activity, Visitors and members are required to acknowledge their acceptance of the Risks & Obligations, as follows:
This acknowledgement of risks applies to all club activities I (we) may undertake as a member of Peninsula Bushwalking Club Inc (the Club). In voluntarily participating in activities of the Club, I am (we are) aware that my (our) participation in such activities may expose me (and my family) to hazards and risks that could lead to injury, illness or death or to loss of or damage to my (our) property. I also acknowledge that I (we) may encounter weather conditions that could lead to hypothermia and being in locations where evacuation for medical treatment may take hours or days.
In particular when participating in abseiling or above the snowline activities I am (we are) aware that these activities could expose me (us) to additional hazards and risks described to me (us) by the activity leader.
To minimise risks I (and my family) will endeavour to ensure that
Each activity is within my (our) capabilities
I (we) will carry food, water and equipment appropriate for the activity
I (we) will advise the activity leader if I am (any of us are) taking any medication or have any physical or other limitation that might affect my (our) participation in the activity
I (we) will make every effort to remain with the rest of the party during the activity
I (we) will advise the leader of any concerns I am (we are) having, and
I (we) will comply with all reasonable instructions of club officers and the activity leader.
I (we) have read and understand these requirements. I (we) have considered the risks before choosing to sign this acknowledgement of risk. I (we) still wish to join the activities of the Club. I (we) acknowledge that I (we) will take responsibility for my (our) own actions and that signing this form and the payment of my (our) subscription will be deemed as full acceptance and undertaking of the above conditions.
